1.    Emergency Department Diversion:

● Must be 18 years of age or older with a  diagnosis of serious mental illness

● Be Medicaid eligible (150% FPL or below)

● Reside three or more (cumulative or consecutive) years in an inpatient psychiatric hospital  in the last five years

2.    Jail Diversion:

● Must be 18 years of age or older with a
diagnosis of serious mental illness

● Be Medicaid eligible (150% FPL or below)

● Two or more psychiatric crises (inpatient psychiatric hospitalizations and/or crisis episodes requiring outpatient mental health treatment), and three or more arrests in the last three years


3.    Emergency Department Diversion:

● Must be 18 years of age or older with a diagnosis of serious mental illness

● Be Medicaid eligible (150% FPL or below)

● A history of psychiatric crisis (inpatient psychiatric hospitalizations or outpatient mental health crisis episodes), and fifteen or more emergency department visits in the last three years.
